M.S. Electronic Coursework Form (LOGIN- must connect to VPN)

Responsible Conduct in Research (M.S. Thesis students must complete one form of RCR training)

Request for Approval of Master's Thesis Topic (DocuSign)

M.S. Certificate of Thesis Approval (DocuSign) - form must be approved by ALL committee members at the time of the thesis.

M.S. Thesis Evaluation Form (Qualtrics) - to be filled out by EACH member of the M.S. thesis committee at the time of the thesis.

Doctoral Student Forms & Guidelines

Ph.D. Proposal

ECE Ph.D. Proposal Committee Form (LOGIN- must connect to VPN) - online form to be initiated by the student

Disclaimer Statement  (DocuSign)- to be inserted inside the proposal document.

Ph.D. Admission to Candidacy Form (DocuSign) – submit the day before the oral presentation. Use Daniela Staiculescu (or the person designated in her absence) for the School Chair and Graduate Coordinator fields.

Committee Chair Report for Ph.D. Proposal (Qualtrics) - to be filled out by the Ph.D. proposal chair the day of the proposal oral exam.

Individual Evaluation Form for Ph.D. Proposal (Qualtrics) - to be filled out by EACH member of the Ph.D. proposal committee the day of the proposal oral exam.

Ph.D. Defense

Dissertation Deadlines

Reading Committee Form (DocuSign) - needs to be submitted to the Academic Office no later than two weeks before defense date.

Schedule Defense Form (Word Doc) – needs to be submitted to the Academic Office no later than two weeks before defense date.

Certificate of Thesis Approval (DocuSign) - submit the day before the dissertation defense. Use the Ph.D. Advisor in the “Chair, Thesis Reading Committee” field, and Daniela Staiculescu (or the person designated in her absence) for the School Chair/Graduate Coordinator field.

Ph.D. dissertation evaluation form (Qualtrics)– to be filled out by EACH member of the Ph.D. defense committee at the time of the dissertation defense.
